How to Request a Temporary Handicap Parking Permit

If you require a temporary handicap parking permit (not to exceed 90 days), proceed as followed: 1. Call the Office of Diversity and Equal Opportunity at 544-4079 or 544-0022 to make the request. 2. Print and complete this Issuance Form. 3. Obtain verification/authorization from your Physician. (verification of care, nature of injury and duration of the parking permit) Have that information faxed to the ODEO at 256-544-2411 4. Bring the completed Issuance Form to the Office of Diversity and Equal Opportunity (Building 4200/Room 420) *If you have mobility issues, arrangements can be made for an ODEO representative to visit your area. 5. After the physician verification is received, MSFC Form 3755 will be issued by the ODEO Disability Program Manager 6. Place this card in your vehicle (driver's side window) which will allow access to any handicap parking at Marshall with the exception of RESERVED handicap parking spaces. Parking in a reserved handicap parking space is strictly prohibited.
